Explanation of Insurance Coverage:

When you schedule an appointment with us, we will verify, to the best of our ability, your vision and medical plans. We do this because we like to have all the necessary information on file in case a routine visit has a medical diagnosis and needs to be filed medically. All information about your benefits is determined through the carriers’ website or by phone and is not a guarantee of payment. Ultimately, although we try our best to verify your coverage, you are responsible for knowing your benefits and for payment.
